1. If your organisation is a church, please confirm that you are representing the select vestry/rector’s views.
2. If you are not representing a church, please confirm that you are representing your management committee’s views.
3. If your organisation is not a church please give the following information:
a) what is the status of your organisation eg voluntary, community, charitable?
b) Is it part of a larger organisation? If so, what one?
c) Is the organisation run by a committee? If so, please list name, address, occupation and position on the committee of each member.
d) If there is an association agreement or constitution, please forward a copy to CREED.
